The Philadelphia Cognitive Assessment (P-Cog) is a concise, four-minute clinical tool designed to aid in the screening and diagnosis of dementia. Intended for administration within a clinician’s office setting, it can be billed under CPT code 96120. The assessment has been validated for detecting dementia and comprises 30 questions that evaluate multiple cognitive domains, including verbal and visual memory, language, logical reasoning, frontal executive functions, mathematics, and visuospatial skills.

Following the test, an expert system provides a score-based interpretation and records potential confounders. This system also suggests possible management plans, which can be printed or digitally integrated into electronic medical records.

The P-Cog generates a comprehensive medical report compliant with CPT code 96120 requirements: “neuropsychological testing administered by a computer with qualified healthcare professional interpretation and report.”
